J-004, How to Correctly Fill Out the Emergency Equipment Shift Ticket, OF 297 (PSM 004)

The Emergency Equipment Shift Ticket, OF 297, is the document used by cooperators and contractors to record equipment use based on the rate(s) assigned in the agreement or contract.  Most incident personnel need baseline knowledge on how to complete the Equipment Shift Ticket and some, such as supervisors and time recorders, have additional responsibilities and knowledge requirements.

This Performance Support Module provides information on:

  • How to accurately complete, authorize, and review the Emergency Equipment Shift Ticket.
  • Ensuring equipment recorded matches what was requested on the resource order and cooperative agreement or contract.
  • Emergency Equipment Shift Ticket responsibilities for cooperators and contractors.
  • Emergency Equipment Shift Ticket responsibilities for supervisors and time recorders.

The Logistics/Equipment Management Committee (LMC) has released Equipment Advisory 26-001: Potential Front Driveline Joint Failure on 2018 and Earlier Ram Pickups. This advisory highlights multiple U.S. Forest Service regions reporting front driveline joint failure incidents.

This advisory recommends firefighters and fire managers to: 

  • Follow all Ram-issued inspection, maintenance, and service guidance for front driveline joints. 
  • Consider installing a driveline loop. 
  • Coordinate with fleet managers and service providers to determine the most appropriate driveline loop kit for each pickup.
